Vitamin B6 decreases the therapeutic effect of levodopa because it acts as a cofactor which enhances peripheral conversion of levodopa to dopamine so that less is available for transport into the brain. Dopamine antagonists such as the phenothiazines, butyrophenones and reserpine (which depletes dopamine stores) counteract the effect of levodopa. MAO inhibitors prevent dopamine breakdown, increasing dopaminergic effects unpredictably. leading to a hypertensive crisis or hyperpyrexia. This is a dangerous interaction.
